Question For Doctors Failed Sleep Attempt?

I am not asking for medical advice. Just asking what happened.

I’ll explain all the steps to give context.
So I was feeling really tired one day and so I decided to have some alcohol (I heard it increases the effectiveness of sleeping pills) I then later took 2 pills of prescribed Melatonin (2 mg each), but after a couple minutes I was impatient and took a 3rd with orange juice. And then after I started to feel the effects I went to bed and tried to sleep, after a while I passed out and then woke up 2 hours later (I checked that I did not miss a day) I felt hot, sweaty and had a bit of a headache, my throat also felt weird and I also felt a bit of acid, I then drank a lot of water to try and feel better. I then couldn’t get back to bed despite still having jelly legs, so I played around until I eventually fell asleep and then woke up at 7:30 am still extremely drowsy and then slept until 2:30 pm.

Notes:
I have tried alcohol with melatonin before but with just one pill and it worked fine.
I think the orange juice caused the acid reflux.
I normally have 2-4 mg of Melatonin before bed.
I initially went to bed at 9 pm which is quite earlier then I normally go to bed.

TLDR: why did having 6mg of Melatonin with alcohol not help me fall asleep?

Need Help Identifying My Mother's Foot Condition (Not Improving Despite Treatment)

My mother is 49 years old and has a history of high blood pressure and type 2 diabetes, but her blood sugar is well controlled (recent HbA1c around 5.7%).

About a week ago, she developed redness, warmth, swelling, and pain in one foot. Her doctor diagnosed it as an infection and prescribed antibiotics and injections. We have followed the prescription exactly as instructed.

However, instead of improving, the redness and swelling have now started appearing in the other foot as well. Both feet are painful, warm to the touch, and swollen.

Her recent blood tests did not show a significantly elevated white blood cell count, and her kidney function appears normal.

We are worried because:

  • The symptoms are not improving despite treatment.
  • The problem has spread from one foot to the other.
  • The skin is red, warm, swollen, and painful.

Could this still be cellulitis, or should we be considering another diagnosis such as venous insufficiency, vasculitis, lymphedema, DVT, gout, or something else?

Has anyone seen a similar case? What additional tests (such as CRP, ESR, Doppler ultrasound, blood cultures, etc.) would you recommend discussing with her doctor?

Any advice from doctors or healthcare professionals would be greatly appreciated. I am not asking for medical advice.

Question For Doctors 48F - Severe Left Knee OA with bone-on-bone contact. 2 TKR recommendations. Questions on conservative options vs. surgery.

Patient Info: 48-year-old female

Primary Complaint: Severe left knee joint pain, reduced mobility. Right knee also affected, but left is significantly worse.

Imaging / Findings: Recent X-rays reveal severe cartilage depletion with clear bone-to-bone contact in the left knee.

Current Medical Opinions: 2 orthopedic surgeons have reviewed the X-rays and recommended a Left Total Knee Replacement (TKR).

Questions:

1) Given her age (48) and bone-on-bone state, is TKR effectively the only definitive solution, or are there any legitimate conservative management protocols that can delay surgery without causing further joint/muscle degradation?

2) Family members have suggested traditional heat/oil therapies (like Ayurvedic Janu Basti). From an orthopedic/evidence-based perspective, is there any structural benefit to these treatments for Grade 4 OA, or are they strictly temporary symptom relief?

3) For patients undergoing TKR in their late 40s, what are the key factors to consider regarding implant longevity and recovery outcomes?

Question For Doctors Lower right stomach pain

Hi ive been having a medical mystery thing for 6 months now and still at a loss as to what it is. In feb it started hurting on my right lower tummy when I wee,cough, have a bm or laugh. It’s in just one spot and comes and gos in pain but is always there. im also going to poo like 7-8 a day which is driving me insane. Doctors have done an ultrasound and checked all my bloods and still have no idea. I am not asking for medical advice. just wondered what I should ask the doctors next as the pain just hasn’t gone. Question For Doctors Stitches hurt so bad I cant sleep. Normal?

Hello,

Recently got 16 dissolving stitches in my ear to provide pressure on a drained hematoma. The swelling is insane and it almost feels like the stitches are cutting into me. It hurts so bad at night but not during the day somehow.

Do you have any advice to help me reduce the pain? Ice helps a bit but it also causes pain and is hard to hold there.

Is it normal to cut someones ear open, provide stitches, and then send them home with no pain meds?
