How is my disease that plays a large part in my story?

IDK who the artist is but the art it is based off of is Kairyu Crocodile.

This is for my fantasy story and like I said in the title it is a large part of my story. One of the two main characters has this and her orphan step brother (the first main character) and it is basically his whole motivation to enter this life and death tournament to make a wish to change her back. This is because the world doesn't really like them and so her life is being threatened (especially since they live in a enlightenment era world) and so he wants to save her.
