Hulk and Banner have combined, separated, reconciled, died, resurrected, become immortal, been seperate personalities, been multiple separate personalities, been separated into two distinct physical people, depowered, and repowered dozens of times. Professor Hulk, Doc Green, Planet Hulk/World War Hulk, and Immortal Hulk were all stories where Bruce and one of his hulks either combined or were acting in lockstep with each other.

I think it's best to view comic continuity as fluid. Or like some kind of hyper-continuity. When you pick up a Hulk comic, Bruce might be any combination of dead, depowered, married, separated, primarily savage hulk, Joe Fixit, Devil, worldbreaker, on the run, or actively part of a team. Just roll with whatever the status quo is during the specific comic you're reading, putting things on a specific timeline or whatever is an endless headache. This goes for any Marvel/DC comic, btw. Alfred has been dead in main continuity for years, but still shows up in stories all the time, and you just have to accept that "Oh, Alfred was alive for this one, cool."

Uh, depends on the comic run tbh. The Immortal Hulk handled it best by representing the 'Hulk System' as a realistic expression of DID with gamma powers. The savage Hulk is a child like temper tantrum that Bruce formed as a child to escape/cope with an abusive father but when he got gamma powers that became the Hulk. There have been a lot of other Hulk personas through the years which are all separate split personalities that were created by various traumas in Bruce's life.

Gamma energy. Gamma in Marvel comics is the nexus/bridge of science and magic. Which is why it gets so fucking weird and magic/science doesn't affect the hulk in ways that it should.

Not really, but one theory is that the moment everyone forgot, Bruce Banner was the active consciousness. So Hulk, being dormant, didn't forget.
