How on earth do I lower my HDL?

46 year old woman, healthy weight, exercise 6 days a week (cardio & strength).

HDL: 108

LDL: 102

Triglycerides: 52

ApoB = 76

Lp(a) = 12

Blood Pressure: 113/72

A1C: 5.1

Glucose: 74

I know that high HDL is considered risky according to new guidelines, but how on earth do I lower it? I stopped taking birth control last year, which dropped my HDL from 134 to 96 - but it's back up over 100 again.

I have maybe 10 drinks a year, so it's not because of alcohol. No one in my family has high HDL. The only medication I take is spironolactone and a topical eczema cream. I already eat a balanced diet, and I'm certain that's not the cause. So then how on earth do I lower my HDL?! Start smoking and stop exercising? Or would you just not worry about it? None of my parents or grandparents lived over 73. Do I just accept my fate?

I am a very healthy 45F, 5'5" and 133. I run 30ish miles a week, weight train 3 times a week and have done all of this my entire adult life. My LDL had been creeping up for years but was fine when I was younger (diagnosed with hypothyroidism 4 years ago).My Mom had a triple bypass at 54 and has continued to have a dozen stents, angioplasty, carotid cleaned out, and most recently a valve replacement. To say she has a horrible history is an understatement. That leads to me and what you all would do. I am on zetia for the last 6 months, I take 5mg daily. Also been doing psyllium husks and trying to keep saturated fat around 12-14g a day. Red meat maybe twice a month. I track all my calories and mostly follow Mediterranean diet.I had a CAC in May and score was zero. Recent bloodwork was apob 83, ldl 86 (down from around 130), hdl 51, total cholesterol 157, triglycerides 90, lpa 132nmol, hs crp <.20, lp pla2 98, ldl pattern A, ldl particle number 1057, ox ldl 42, apoe 2/3. I also have great BP 110/70 I go to a functional medicine doctor and they've ran everything under the sun. I know my lpa is insanely high. They prescribed the zetia back in January when I asked for it. Based on my current numbers what would you do. If I had a CAC of anything other than zero I would have added a statin but it seems almost unnecessary at this point. Thoughts?

26M with Lp(a) of 214 nmol/L

Recently got my function health results back and my Lp(a) was pretty bad.

Other numbers:
ApoB: 87 mg/dL
HDL-C: 33 mg/dL
LDL-C: 100 mg/dL

A1C: 5.7 (I’m actively working on fixing this with fasting and eating better)

I’ve also recently decreased my visceral fat from 1.34lbs to 0.78lbs

I’ve also starting talking fish oil aggressively and after doing that my trigclerides have gone from 84 —> 35 which was good.

I’m cleaning up diet and trying some other lifestyle fixes. I lift 3x a week pretty consistently but I have struggled with a bad sweet tooth.

I’m also thinking about seeing a preventive cardiologist but maybe that’s a waste of money? Anyone have experience with that?

Is getting a CAC now a waste of money?

My dad also struggles with this (he’s now a type 2 diabetic, so was his mother) but he has a poor diet. His CAC when he was 52 was 169 and has been on a statin since then which I really want to avoid.

Losing body fat; raised LDL and APOB.

My recent lab work from January, two months ago, and yesterday are as follows.

January: LDL 93 , triglycerides 75 , Vldl 12 , APOB 71 mg/dL.

- I was eating more carbs and somewhat healthy but still a bit more carbs than what I’m doing now and two months ago.

2 months ago: ldl 126, triglycerides 61, Vldl 10, APOB: 106

(Big increase) when I was losing body fat.

Now: LDL 96, APOB 87, triglycerides 52, Vldl 7.

(I’m dieting to lower carbs and less food now to lose body fat).

In January I was not dieting , I weighed 150 lbs at 5’8 24 year old male and 18 % body fat. I was still eating healthier than your average American diet but not like now.

Now: I’ve lost body fat and gained muscle, I went to 12% body fat and constantly burning fat more than carbs for fuel. Why would my APOB be higher now than when I was eating less healthy… on top of my triglycerides being extremely low now. I thought when u ate healthier your apob and LDL was supposed to lower, not raise. (I don’t eat any red meats, sugar, butter, or cooking oils. Just avocado oil and extra virgin olive oil now.

Does this mean my body is releasing more cholesterol into the bloodstream because I’m burning more fat for energy? Someone help please!

What Next?

50F Total 190, HDL 75. Just got l(p)a tested through Function last year and it was high 121. Decided to get a CAC. Unfortunately it came back 109. Great metabolic markers and normal blood pressure. I’m waiting for response from cardiologist but I want to be as aggressive as possible. I want a lot more time on this side of the ground. What should I advocate for?

Impact of "rather low calorie" days?

Recently getting started w fat loss w a different "strategy" than I have in the past, using calorie counting (paying attention to protein/macros as well, ofc). Feels like its going well so far (just a few weeks in). Yesterday I basically had a fairly low calorie day and it just felt quite fine w surprisingly no...obvious negatives? Energy, mood, etc, all seemed fine. Might impact my clarity of thinking or focus on a work day, I'm not sure, but, at least for a weekend, was OK.

I've been hitting 2650 calories per day, a slight deficit, and losing some weight on that. Yesterday I had just 1000 calories and ended up feeling fine. Based on what I've eaten today, I could see hitting just 1000 calories today as well and feeling fine. (Not going that low today, just going off of how I feel.)

Even with 2650 calories, which supposedly should just be a small deficit, I seem to be losing weight, a few weeks in. I'm fairly high protein (0.80-1.00 g/lb which, given my high body fat percentage, should be more than sufficient from my reading) and on the low calorie day yesterday, quite biased towards protein.

Is there much of a problem w these low calorie days, once a week or so?

Feedback 34F (almost 35) — Why am I not building muscle despite years of strength training? (Dexa)

I’m a 34-year-old woman (turning 35 soon), and I’m getting really frustrated with my lack of muscle growth.
I’ve been strength training for years, but if I’m honest, I haven’t always been consistent. Over the past several months, I’ve been much more consistent and recently started Bunda, which combines lower-body strength training with high-intensity workouts.
I recently compared my body composition to my 2023 scan, and it looks like I’ve gained little to no muscle. According to the results, around 80% of women my age have more muscle than I do, which was honestly discouraging.

Unsaturated Fat vs. Saturated Fat & LDL/APOB

I've been posting a lot and will probably be posting more now that I've got these results. History: 42, F, very healthy, 5'3 120lbs, work out regularly, was eating keto/carnivore-ish for a few years, high Lp(a) and high LDL, excellent trigs and HDL, high APOB, excellent inflammation markers across the board. Just got a cleerly CCTA and I have 31% stable soft plaque in the LAD, I'm in the midst of IVF so I can't start a statin right now. Yes, I'm freaking out.

My question is this: I like eating higher fat. I don't do well with grains and sugars, they just make me want more. I'm totally find switching to unsaturated fat. But if I do that and my overall fat intake is still a high % of calories (ie: 50-60%, aiming for keto but more of a mediterranean low carb approach), is that still going to mess up my LDL and APOB? Or will they actually come down? I'm talking avocado, salmon, olive oil, nuts and seeds etc. Is there any evidence, any n of 1 experience, or do I just need to do it and test my lipids again in 6-8 weeks? Or do I go on a low fat diet? I'm struggling here figuring out what to eat that's not grain/crap heavy, and I like keto-mediterranean but I'm just not sure it will actually lower my LDL and can't find much about it online.

Can it be that positive health outcomes associated with better VO2 max scores are greatly due to healthy user bais?

Peter points out that this is the case with vegan diets: people with restrictive dietary patterns are more invested in their health and, in turn, make better health-related choices across the board, which can lead to positive outcomes. Why can't the same be said for people killing it in the gym?

Has anyone experience negative side effects from Repatha? More context inside. Thanks.

Crestor and another statin both elevated my CK pretty high. Zetia was doing a decent job at maintaining but it elevated my AST/ALT. So I was giving Repatha. Haven’t taken it yet.

One doctor said I’m a “hyper-responder”. For context my LDL was 212 and Crestor 20mg and zeta brought it down to 27. Stopped Crestor and stayed on zetia and it was around 112. Stopped the zetia. LDL is now 218.

Had familial test and it was negative.

I don’t know if I should just go back to zetia and diet better as the ast/alt may have only been elevated due to working out. I don’t drink also.

Or if I should just go to repatha.

I had a Heart CTA with cac in 2024. CTA was negative for anything and CAC was 0.

My LPA is also 7 I think.

Discussion Microdosing Statins?

I’m curious how much someone could lower cholesterol levels by micro dosing Rosuvastatin or Atorvastatin? My LDL cholesterol is already much lower than average without taking any medication (LDL is 65mg/dl and ApoB is 60mg/dl), but I would like to get my cholesterol levels even lower than this to push the date when I cross the 5,000 mg/dl threshold back further. Since I am around 30 years old, I am assuming my current lifetime exposure to LDL is around 2,000 mg/dl. If I continue to not treat my cholesterol levels, I would likely reach this 5,000 mg/dl threshold in sometime in my 60s. I would like push this back significantly, so I don’t reach this threshold until I am around 90-100 years old. It seems like I only need to reduce my cholesterol levels by around 25% from where they are now to accomplish this, so what are the minimum doses for a atorvastatin and rosuvastatin respectively that would accomplish this?

Lab Results Feedback on confusing test results

So I've (34F) dealt with high cholesterol for a few years now and I can't seem to get it to come down. I have a pretty healthy lifestyle, workout plenty, eat a good amount of fiber, BMI is around 20-21, etc. I just got the following test results (all mg/dL and I was fasted):

HDL: 97

LDL: 129

Trigs: 172

VLDL: 29

ApoB: 110

LipoA: 14.4

Meanwhile my blood pressure was 102/54 and my hemoglobin A1c was 4.9%. I just don't know how to explain these findings... like why is my HDL so high? How are my trigs high but VLDL low? Why are several of my cholesterol markers high despite having no other signs of metabolic syndromes? I'm not sure if anyone has advice for me or any thoughts here - I'm willing to make more lifestyle changes I'm just a bit confused and would appreciate some perspectives.
